Public incident reports record what happened, its impact, causes, and follow-up work. [Architecture decisions](../adr/index.md) record design choices and their alternatives.

## Conventions

- Add reports here as `YYYY-MM-DD-short-title.md`, using the incident date. Keep filenames stable and link each report from this index.
- Review reports and updates through pull requests. Record affected versions, impact, timeline, causes, recovery, and follow-up work; link related issues and ADRs.
- Date later updates and keep unresolved follow-ups explicit. Exclude credentials and private customer details.
