Conventions

• Add reports here as YYYY-MM-DD-short-title.md, using the incident date. Keep filenames stable and link each report from this index.
• Review reports and updates through pull requests. Record affected versions, impact, timeline, causes, recovery, and follow-up work; link related issues and ADRs.
• Date later updates and keep unresolved follow-ups explicit. Exclude credentials and private customer details.
